Use of medication can lead to side effects. You can find the known side effects listed in the patient leaflet of your medication. We welcome you to inform us in case you have experienced one or more side effects when using our medicines. You can also report side effects to the National Adverse Events Center Lareb in the Netherlands: www.lareb.nl

The lot number can be found on the strip and / or packaging at the expiry date.
  • All information is treated in strict confidence.
  • Your personal data will only be used for registration and analysis purposes, as well as the effective handling of your possible complaint (s). To properly assess your report, additional information may be requested by telephone or e-mail.
  • The reports of side effects that we receive are processed with the aim of being able to monitor drug safety both nationally and internationally.
  • I (the reporter) am 16 years or older.
